Members simply choose the most convenient participating vision providers from our Database
Members then contact Member Services to set up an appointment with a participating provider of their choice.
Members present their ID Cards each time they visit and pay the participating provider the special member rates directly.
Participating providers are subject to change without notice. Not all chain locations under the companies listed above are contracted participating providers. Call Customer Services to ensure your desired location is in-network.
*Prescription must be filled by the provider performing the no-charge exam if glasses are required. NOTE: Network providers may NOT be available in certain areas.
NOTE: Member is responsible for all applicable payments at point of service. Any procedures performed by a Non-Participating Provider are not eligible for discount. Members are advised to contact a Customer Care Consultant to verify that their chosen eye doctor is a Participating Provider prior to their visit.
